Vanessa Zwegers, FNP-C

Vanessa Zwegers, FNP-C, is a dedicated primary care provider with a strong background in emergency medicine and a passion for helping patients improve their overall health. She began her healthcare career as an emergency room nurse in Pittsburgh, bringing both clinical skill and compassion to high acuity care. After four years as a nurse, she advanced her education at Robert Morris University, becoming a nurse practitioner in 2013.

Originally from Canada, Vannessa moved to Alamosa in July 2012 and has since built meaningful connections within the community. She previously practiced primary care at Valley Wide and has a particular interest in addiction medicine, having worked closely with Hope in the Valley. She is especially passionate about addiction counseling and helping patients manage chronic conditions such as diabetes.

Vanessa takes a whole person’s approach to care, focusing not only on the individual but also on the role of family and support systems in long term health. She believes that building trust starts with being genuine, meeting patients where they are, and creating a comfortable environment where they feel heard and supported. One of the most rewarding parts of her work is seeing patients overcome challenges, especially those working toward recovery from addiction.

She was drawn to SLV Health by the opportunity to maintain a strong work life balance while serving her community. Vannessa values caring for her neighbors and is committed to helping patients reach their health goals through trust, collaboration, and personalized care. She wants every patient to leave an appointment feeling comfortable, motivated, and confident in their next steps.

Outside of the clinic, Vanessa enjoys running, playing hockey, and spending time with her family. She is actively involved in her daughters’ sports and coaches hockey, reflecting her lifelong love of the game, which began when she was recruited to play in college. She is also happy to call the San Luis Valley home, where her parents have retired.
