SLV Health’s Conejos County Hospital (CCH) is located 25 miles south of Alamosa on Highway 285. CCH is a 17-bed critical access hospital and primarily serves residents of Conejos and Costilla counties. It was built in 1963 with generous donations from local families. Laboratory –719-274-6021 - Monday-Friday, 8:00 a.m. – 5:00 p.m., offering Direct to Consumer testing. Fully staffed and accredited! For forms and more information, go to the lab page.
Radiology & Imaging – 719-274-6029 –Monday - Thursday services are available 24/7. Friday 8 am to 11 pm. Saturday and Sunday 10am to 10pm. We offer Xray/CT and limited ultrasound services at CCH. For all radiology services offered at SLVH, visit their page.

Sleep Lab – 719-587-5764 - Sleep Studies are available by appointment on Tuesday, Wednesday, and Thursday. Sleep study hours are 6:30pm to 7:00am. The types of tests done are Polysomnography (PSG), Split Night and CPAP/BIPAP titration. Read more about the SLVH Sleep Lab here.
- We also partner with Rocky Mountain Children’s Hospital (RMCH) for pediatric sleep studies. These are done 2 days a month, the last Monday & Tuesday of the month, with referrals from the SLVH Pediatrics Providers.
Swing Bed Services – Contact our discharge planner at 719-274-6011. Swing bed is continued skilled nursing and rehabilitation services as the patient continues their recovery. Swing Bed is generally a Medicare Benefit that follows a three-day qualifying stay in an acute care setting in the past 30 days.
Emergency Department - The Conejos County Hospital Emergency Department is a 6-bed emergency department staffed by advanced life safety providers. The Emergency Department is equipped with both a triage room and trauma bay for critical care and trauma patients. It is open 24-hours a day for medical emergencies.
Inpatient Services – Conejos County Hospital maintains a 17-bed inpatient unit to help meet the inpatient medical and post-surgical needs of our community. Our inpatient services are staffed by our clinical team trained in advanced life safety. The team is led by our Hospitalists providers, a group of physicians, and advanced practice providers trained in inpatient medicine and hospital-based services. Financial Services – Please call 719-587-6364. A Patient Financial Counselor at CCH is available Monday, Wednesday, Thursday, and Friday, 7:00am to 5:30pm. Primary Care may be obtained in Conejos County in either the La Jara Clinic or the Antonito Clinic.
