Dr. Blake Reichert Joins SLV Health as First Rural Training Track Graduate Placement

One of San Luis Valley Health’s newest Primary Care providers, Dr. Blake Reichert, DO, is also the first placement of the Rural Training Track (RTT) Residency Program. Launched in 2016, the RTT is a collaborative effort between SLV Health, Valley-Wide Health Systems and Southern Colorado Family Medicine Alamosa Rural Training Tract, that allows Family Medicine Residents to live and work in a rural community while receiving medical education and training from an accredited residency program.
The RTT was established to address rural physician workforce shortages and give residents exposure to practicing in a rural setting with the hopes that they would choose to practice in the San Luis Valley. Dr. Reichert became the first successful placement of the RTT when he began with SLV Health in September 2025.
Why Rural was the Right Fit
According to Dr. Reichert, “The Rural Training Track was my first choice for residency. I really wanted to check out Southern Colorado and the San Luis Valley.” While the mountains and the endless outdoor recreation opportunities of the San Luis Valley were large factors in Dr. Reichert deciding to establish his practice here, “What I’m most excited about in this new role is the opportunity to build long-lasting relationships in the community.”
SLV Health is equally as excited to welcome Dr. Reichert to the team. SLV Health CEO Konnie Martin stated, “Welcoming our first physician from the Rural Training Track is a proud milestone for both the program and our community. This program was designed to educate future physicians where they’re needed most, and we’re so excited to have Dr. Reichert join our team! We hope that this is the first of many successful placements from this residency program.”

Residents in the RTT spend their first year in the Southern Colorado Family Residency Program at St. Mary Corwin Hospital in Pueblo, then two years in the San Luis Valley, seeing patients at the Valley-Wide Alamosa Family Medical Center clinic and at SLV Health’s specialty clinics. Dr. Reichert graduated from the program in June and is now accepting patients as part of SLV Health’s Primary Care team.
