- Additional Specialties: Cardiology, Surgical Services
Tylen Pavlovsky, PA-C, is a dedicated member of the cardiology team at San Luis Valley Health. His commitment to cardiac care is deeply personal, rooted in seeing the impact of cardiovascular disease within his own family. This early interest led him to work in the cardiac unit at St. Mary’s Medical Center in Grand Junction before pursuing his advanced medical education.
A fourth-generation Valley native, Tylen graduated from Centauri High School and earned his Bachelor of Science cum laude from Colorado Mesa University. He received his Master of Physician Assistant Studies from the Creighton University School of Medicine in Omaha, Nebraska. During his time at Creighton, Tylen was awarded the Ronald Dieck and Erin McGurk Scholarship and the 2022 Heart of Service Award, recognizing his exceptional commitment to community service.
Tylen brings a broad clinical perspective to his practice, shaped by previous training in various healthcare systems throughout Nebraska, Arizona and training at the Heart and Vascular Center of Arizona. His care philosophy centers on evidence-based medicine and patient empowerment. By helping patients understand the "why" behind their diagnosis, he strives to ensure that they are active partners in choosing the screenings, medications, or lifestyle changes that best align with their personal health goals.
Outside of the clinic, Tylen is a Peloton enthusiast who enjoys fishing, coaching wrestling, and cheering on the Broncos. He loves spending time with his wife, Carmen, also a Valley native, and their daughters, Khaleah and Khloe, as well as spending time outside and enjoying the natural beauty of Southern Colorado.
