Resetting Your Username and/or Password
You can reset your own log in information by going to the patient portal log in screen and selecting the option that fits best. You will be allowed 3 attempts before getting locked out. If you have been locked out, contact SLVH's Customer Service team at Tegria by calling (719) 587-1392 and Press 2, or email them at sanluisvalleypatientportal@tegria.com, Monday-Friday, 7:00 am - 10:00 pm.
By clicking Forgot Logon ID? you can reset your own username and password by entering your email address and submitting a request to reset credentials. You will receive an email from “Your Trusted Partner in Health.” Open this email and follow the directions.
By clicking Forgot Password? you will be able to reset your password, without resetting your log on ID. By entering your Logon ID and email address, our system will send you an email to reset your own password. Open the email and follow the directions.
